This webpage provides original investigations of collapses and other incidents conducted by the Directorate of Construction, OSHA.
Many of these incidents resulted in one or more worker fatalities, and most of them resulted in multi-million dollar property loss, lawsuits, or settlements. Due to the nature of investigations of construction collapses and other incidents, these reports may contain professional opinions of the investigating engineer, opinions of incident root cause, and factual data and findings. Our goal is to help employers, workers, construction engineers, project managers, and regulatory bodies identify problems in construction design, project management, and management of field engineering changes.
Hopefully, this information will help reduce future incidents, fatalities, and serious injuries. Some of the photographs, schematics, computations, tables, figures and other non-text items have been removed from the posted versions and are not displayed. The text of each report is otherwise identical with the original document.

Eye-witness Ivan, 37, evacuated on Tuesday from the nearby building where he works, described the collapse as unbelievable. It is in continual maintenance. As cranes moved in to shift truck-sized chunks of broken concrete, hundreds of firefighters searched for survivors, while public shock and grief turned to anger over the state of the 1. Salvatore Lorefice, 58, a pensioner who lives a few hundred meters yards from the bridge, said cement had fallen off the structure as early as the s when he worked at a warehouse directly under the bridge.
Fire brigade spokesman Luca Cari said firefighters were at the site, helped by cranes that cleared away large rubble and created spaces for rescue teams to check for survivors. Toninelli earlier said he had begun a process to strip Autostrade of its concession and he demanded top managers at the firm resign. The Morandi Bridge, named after the engineer who designed it, forms part of the A10 motorway run by Autostrade. The km 34 mile stretch of the A10 accounts for around 1.
He also said the government would inspect the structure of ageing bridges and tunnels across the country with a view to launching a program of remedial works if required. Within hours of the disaster, the anti-establishment government that took office in June said the collapse showed Italy needed to spend more on its dilapidated infrastructure, ignoring EU budget constraints if necessary.

Brad Waybright It's all okay as long as it's okay. Structural weakness versus structural instability is a meaningful distinction. Don't know if they're right here.
Was it a cable-stayed bridge? The video seems to show a tower collapsing. Approximately ten vehicles were reported to have been on the collapsed span. The initial hypothesis formulated is that of a structural failure.Experts have said a bridge which collapsed during a storm leaving at least 26 people dead in Italy must have been flawed in construction or suffered wear and tear over time.
Dozens of vehicles were sent plummeting ft into a river and onto grassland when the Morandi Bridge came crashing down in the northern city of Genoa on Tuesday morning. Witnesses said the road bridge was hit by lightning moments before it collapsed, but experts have since said this would not have prompted the disaster. Agathocles Giaralis, deputy director of the University of London's Civil Engineering Structures Research Centre, said it was more likely the bridge had flawed foundations.
I don't see how that would be possible as it's reinforced concrete and it's certainly never happened before. Dr Giaralis told the news website that the bridge, which was completed in the s, may also have suffered from extensive corrosion.
Autostrade, a unit of the infrastructure group Atlantia, manages the section of the toll highway that collapsed. The bridge was constantly monitored and supervised well beyond what the law required," Stefano Marigliani, the Autostrade official responsible for the Genoa area, said.
The firm said it would work with investigators, and that it was already assessing how to rebuild the bridge. It said work to reinforce the slab under the road surface was going on at the time of the collapse. Residents have also said how they were worried for a long time that the bridge was not safe as it emerged the design of the structure had been heavily criticised in the past.
Nobody has ever crossed that bridge with a light heart. Italian media reported that 35 people were feared dead with at least 20 fatalities confirmed by the Italian Civil Protection Agency.
Horrifying footage showed an 80m swathe of the bridge plunge 90m onto a road, railway tracks, buildings and a river below. The design of the bridge has been criticised in the past. Antonio Brencich, a professor specializing in reinforced concrete construction at the University of Genoa, called the span "a failure of engineering" in an interview in Sooner or later it will have to be replaced.Previously unseen footage shows moment of Genoa bridge collapse
I do not know when. But there will be a time when the cost of maintenance will be higher than a replacement," he told Italian media Primocanale. The CNR civil engineering society called for a "Marshall Plan" to repair or replace tens of thousands of bridges in Italy that have surpassed their lifespans, having been built in the s and s with reinforced concrete. It said the bridges were built with the best-known technology of the time, but that their working life is 50 years.
In many cases, the cost to update and reinforce the bridges is more than it would cost to destroy and rebuild them, it said. Other engineers said corrosion or weather conditions could have been part of the reason for the collapse. Devastating images showed concrete slabs on the roofs of factories and buildings while helicopter footage showed the horrific aftermath of the tragedy. One poignant photograph showed a green lorry stopped just short of the point of collapse while further video showed injured victims being airlifted from the scene.
Almost rescuer workers were sifting through rubble in search of survivors after at least 10 cars plummeted from the bridge around Between 30 and 35 cars and three heavy trucks were involved in the tragedy including those crushed by huge slabs of falling concrete, officials said. The 1. The motorway it carries is a major artery from northern Italy's industrial centres to the Italian Riviera and to France's southern coast.

However, crane tipping accidents frequently occur with loads below the threshold of the crane.
For example, Crane Tech posted an article about a mobile crane tipping over from only a couple hundred-pound electric motor. What caused the crane accident?
The crane operator failed to fully extend the crane outriggers properly during crane setup. Here are some quick tips for setting up the crane prior to making any crane lift:. Whether the job consists of lifting a couple tons or lifting a couple hundred pounds the mind set should always be the same. Always ask questions before a crane lift.
